EL- Aged Care Ltd/ GTE, an NGO working on integrated community care and support for older persons residing in remote and rural communities in Nigeria. We are currently working on a pilot project to implement a “Chronic Disease Self-management Intervention” that will educate and assist clients that visit our Adult Day Centre make informed decisions about care, strengthen competence and health behaviour change to prevent disease progression; thus improve quality of life.
